I hate to jump the gun but I have a pretty good idea of what happened and how it should have been handled. 30 seconds before the collision the controller ASKED the helicopter if it had the airliner in sight. The controller at that point had no time to be asking questions. The correct response to the helicopter closing on the intercepting airliner had to be a command to the helicopter to Turn right immediately and to command the airliner to turn right immediately. There was no time for questions, The controller had to assert himself and take control! Bad training? A brain fart? I don't know, but pilots aren't looking at their instruments at 500 foot, they are looking out the window at the runway. The only one who was looking at it happening on the screen was the controller and he or she or it was indecisive!
